For registration, the following is needed:

 

Completed Registration form

Completed Pick up Policy form

Ethnicity Identification Information form

Copy of the student's Birth Certificate

Copy of the student's Yellow Shot Card with the up to date requirements

Student needs a TB or PPD test for the current year

Medical physical examination signed by the doctor, due before classes begin (Your child cannot be admitted to school without these papers Public Health and Social Services Law, and School Policy No. 222)
